EGR Issues

My 1997 Legacy 2.5 GT is getting the P0400 code (EGR Flow Malfunction). I have been tracking the problem and Im not having too many issues there. It seems to be a dirty EGR system (the engine has 215K on it). My question is how does the computer read the flow issue? Where does it get the signal from the engine to throw a CEL? O2 sensors, solenoids?

Sometimes this code can be triggered by a loose gas cap or not tight enough gas cap.
